Vibrance Health’s Family Nurse Practitioner (FNP) provides primary health care services to MEHOP clients, focusing on health promotion, illness prevention and management of stable chronic and minor acute illnesses. The Family Nurse Practitioner will be practicing independently and in collaboration with the Medical Director who will provide direct leadership and direction.

JOB DUTES AND RESPONSIBILITIES:

· Provides direct clinical patient care to patients of Vibrance Health and provides appropriate documentation.

· Performs client health history and examination.

· Assists in developing, implementing and evaluating operating policies and procedures relating to clinical services, in collaboration with the Medical Director.

· Provides guidance to staff and develops protocols with the Medical Director in developing comprehensive plan of care of clients.

· Participates in the implementation of the Quality Assurance Program, Medical Peer Review, and CPI Committee.

· Ensures appropriate orientation and continuing education offerings for self to maintain excellence, including safety and clinical competency.

· Promotes and participates in care team.

· Together with the Medical Director and other Providers, establishes and maintains systems for client referral to Medical Director/Physician and/or other health care providers.

· Maintains accurate record keeping system and client database.

· Participates in Clinic and other collaborating activities associated with professional status

· Maintains membership and participates in activities of professional associations.

· May provide leadership to colleagues in teaching, serve as a knowledgeable resource person to colleagues and community.

· Strives to be an advocate for MEHOP in the local communities and participates in outreach activities.

· Supervises nursing staff to insure compliance and achievement of health care goals.

· Promotes and participates in Care Team.

JOB REQUIREMENTS AND EDUCATION:

· A minimum of five years experience as a RN in a clinical setting and preferred experience as a Family Nurse Practitioner.

· Master of Science in Nursing with Family Nurse Practitioner specialization required.

· Licensure as a Registered Nurse in the State of Texas.

· Certification as a Family Nurse Practitioner with Prescriptive Privileges by the State of Texas of Nurse Examiners required.

· Demonstrate competence in delivery of primary health care services.

· Excellent verbal/written communication skills.

· Demonstrates knowledge of client care delivery.

· Knowledge of nursing practice and the ability to teach students in clinical and classroom setting.

· Awareness of current issues/trends in health care delivery and nursing education.

· Community and professional leadership and networking skills.

SALARY: Commensurate with experience.

Pay: $90,000.00 - $110,000.00 per year
